Privacy Policy

Serenity Haven · Tamara Ayala, Licensed Psychologist

Serenity Haven ("we," "us," or "the Practice") respects your privacy. This policy describes the information we collect through this website and how we use and protect it. It applies to information collected on this site and is separate from the clinical privacy protections described in the HIPAA Notice below.

Information we collect

  • Information you provide: When you call, book online, or otherwise reach out, you may share your name, phone number, email, or other details needed to schedule care.
  • Automatic information: Like most websites, basic technical data (such as browser type or general location) may be logged by our hosting provider for security and performance.

How we use your information

  • To respond to your inquiry and help you take the next step toward care.
  • To schedule a consultation or direct you to the appropriate booking platform.
  • We do not sell, rent, or trade your personal information to anyone.

Please don't share clinical details through the website. This site is for general inquiries and booking only — not for emergencies or sensitive health information. Protected health information is handled through secure, HIPAA-compliant systems once care begins.

Third-party booking platforms

This site links to independent scheduling platforms (such as Headway, Psychology Today, Zocdoc, and TherapyDen). When you click through to those services, your information is governed by their privacy policies, not ours. We encourage you to review them.

Data security

We take reasonable measures to protect information submitted through this site, including secure (HTTPS) transmission. However, no method of transmission over the internet is completely secure, and we cannot guarantee absolute security.

Your choices

You may request that we delete an inquiry you've sent, or ask what information we hold, by contacting us at the phone number below.

HIPAA Notice of Privacy Practices

Summary — a full Notice is provided at intake

This summary describes how your protected health information (PHI) may be used and disclosed and how you can access that information. This is a summary for convenience; a complete Notice of Privacy Practices is provided to you before or at your first appointment and governs in full.

How your information may be used

  • Treatment: To provide and coordinate your psychological care.
  • Payment: To bill and obtain payment from you or your insurer.
  • Health care operations: For quality, administrative, and operational purposes of the Practice.
  • As required by law: Including limited situations involving safety, abuse reporting, or court orders, consistent with Florida law and federal regulations.

Your rights

  • To inspect and request a copy of your records.
  • To request corrections to your records.
  • To request restrictions on certain uses and disclosures.
  • To request confidential communications.
  • To receive an accounting of certain disclosures.
  • To receive a paper copy of the full Notice upon request.

Most uses and disclosures of psychotherapy notes, and uses for marketing or sale of PHI, require your written authorization.

To exercise any of these rights, or to receive the complete Notice of Privacy Practices, please call (954) 793-5573.

Good Faith Estimate

Your rights under the No Surprises Act

You have the right to receive a "Good Faith Estimate" explaining how much your care will cost.

  • Under the law, health care providers must give clients who don't have insurance, or who are not using insurance, an estimate of the expected charges for services.
  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency services, including psychotherapy.
  • You can ask for a Good Faith Estimate before you schedule a service, or at any time during treatment.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or picture of your Good Faith Estimate.

For questions or to request a Good Faith Estimate, call (954) 793-5573. For more information about your rights, visit www.cms.gov/nosurprises.
